Three times a day: e.g. 6 AM, 2 PM and 10 PM.

Twice a day: e.g. 8 AM and 8 PM.

Four times: e.g. 6 AM, 12 PM, 6 PM and midnight.
Every 4 and every 24 hours
Every 4 hours means up to 6 times a day (many fever medicines have a daily maximum — read the leaflet). Every 24 hours means once, at the same time each day.
Antibiotic timing
Antibiotics are best taken at the fixed interval on the prescription (usually every 8 or 12 hours), finishing the full course. In Ramadan, ask your doctor before changing times.
